Bronx-Lebanon’s Department of Family Medicine is a dynamic and progressive program. The department, under the direction of Dr. Doug Reich, Chairman, Family Medicine and Program Director, is committed to providing excellent, comprehensive, and continuous primary care; outstanding medical education; and conducting community based health research.

A new philosophy has blossomed in Family Medicine, which embraces the need to provide a “medical home” for patients and families.

The department is dedicated to providing a substantial impact on the health related needs and issues in the communities and neighborhoods in which Bronx-Lebanon provides medical services.

It utilizes the chronic disease management model in its effort to impact such common health problems, as hypertension, diabetes, depression, and immunizations; empowering patients to manage and be in control of their medical illnesses.Patient-centered care, which is provided at the four Family Health Centers: Fulton, Ogden, Crotona Park, and MBD, results in efficient and user friendly services.

Outreach and collaboration with community-based organizations are central to the mission of the department. The collaboration is in the form of the delivery of health services, research, and the development and expansion of existing programs.
